Understanding Heart Disease and Diabetes in Dogs

While each disease affects very different systems in the body, they often lead pet parents down a surprisingly similar path of care, monitoring, and long-term management. A dog with heart disease may struggle with circulation and oxygen flow, while a dog with diabetes is working to maintain stable blood sugar levels. In both cases, the day-to-day reality often involves routine medications, dietary adjustments, regular veterinary checkups, and close attention to subtle changes in behavior or energy.1,4,5
What makes these conditions especially important to understand together is not just their prevalence, but the way they shape a pet’s overall quality of life and a pet parent’s daily routine.
Early signs of both conditions can be subtle and easy to miss. Slight fatigue, increased thirst, weight changes, or reduced activity may not seem alarming at first, but they can signal the beginning of a serious chronic condition that warrants veterinary evaluation.

Heart Disease in Dogs: Don’t Ignore that Cough!
One of the most overlooked symptoms in veterinary medicine is coughing, because a cough can mean many different things. When a dog huffs, hacks, or clears their throat, many owners assume it’s just allergies, excitement, or a normal part of the aging process; and sometimes it is. But a persistent cough can be the first warning sign of underlying heart disease, and should never be ignored.1
As both a pharmacist and a dog owner who has spent nearly twenty years in veterinary medicine, I’ve noticed that pet owners frequently call me with medication questions; yet they rarely call to discuss things like a new cough. I’ve seen the danger of an overlooked cough play out in my own family (first with my dog Daisy twenty years ago, and more recently with my brother’s dog, Einstein). Because our pets fit so seamlessly into our daily routines, it is incredibly easy to miss gradual shifts in their health. If your dog starts coughing, understanding the cause is vital, but diagnosing it is not a DIY project for internet searches. When you notice any change, even a subtle one, making an appointment for a veterinary examination is the most critical step you can take.
It’s helpful to know that certain patterns repeat themselves across specific breeds. For example, myxomatous mitral valve disease (MMVD) is especially common in small to medium dog breeds such as Cavalier King Charles Spaniels, Chihuahuas, Miniature Poodles, and Cocker Spaniels.1 In MMVD, the valve between the left chambers of the heart thickens and degenerates over time. Instead of forming a perfectly tight seal to keep blood moving forward into the body, the compromised valve allows blood to leak backward into the left atrium. To compensate, the heart muscle must work harder, eventually causing cardiac enlargement. As the condition progresses, pressure builds, and fluid accumulates within the delicate tissues of the lungs (pulmonary edema), making breathing more difficult.
The early signs of these cardiac changes can be incredibly subtle, easily masked by a pet’s natural resilience. Owners typically notice a soft, persistent nighttime cough, mild exercise intolerance during ordinary walks, or more labored breathing during deep sleep that seems a little faster than usual, but notoriously difficult to spot. Dogs are fundamentally wired to hide weakness, meaning the outward signs we observe are often just the tip of the iceberg.
From a pharmaceutical standpoint, managing a canine cardiac patient requires an extremely thoughtful, highly tailored approach. Fortunately, heart disease management has improved tremendously over the years, evolving from basic palliative care to highly sophisticated, multi-drug protocols that actually alter the progression of the disease. When establishing a long-term care plan, veterinarians generally rely on a trifecta of medications to support the failing heart and extend both lifespan and quality of life: pimobendan, diuretics, and, in certain circumstances, ACE inhibitors.
Inodilators like pimobendan (eg. Vetmedin) are widely utilized as a cornerstone of therapy. They work by increasing cardiac contractility and causing vasodilation, reducing the resistance against which the heart pumps. The overall effect is to help the heart work better, whilst simultaneously making its job easier.1,2
ACE inhibitors like enalapril or benazepril act on the RAAS (renin-angiotensin-aldosterone system), blocking the hormones responsible for vasoconstriction, effectively lowering the workload on an enlarged heart and preventing harmful tissue scarring.1,3
Diuretics like Furosemide, Torsemide, and Spironolactone act on the kidneys to promote fluid excretion by increasing urine output. This effectively draws fluid away from areas it has accumulated, which in the case of pulmonary edema in dogs with congestive heart failure, is the lungs. Diuretics are vital in the management of heart disease, offering immediate relief from heavy, fluid-induced coughing spells.1

Timing Is Everything
In the past, medical management of heart disease tended to be more reactive, with medications selected to respond to the clinical signs as they appeared. This all changed in 2016 with the publication of the results of the Evaluation of Pimobendan In dogs with Cardiomegaly (EPIC) Study, which altered the landscape of preclinical management of MMVD. Evidence from the EPIC trial showed that introducing medications like pimobendan during the asymptomatic “remodeling” phase (Stage B2) significantly delays the onset of overt congestive heart failure or cardiac-related death.1,2 This breakthrough study allowed veterinarians to intervene in the earlier stages of canine heart disease, rather than simply watching and waiting.
As someone who has spent years consulting on medication management and counseling anxious pet owners, I always encourage families to develop a close relationship with their veterinarian and take an active role in their pet’s care. As an example, counting how many times your dog breathes per minute while they are completely relaxed or sleeping is one of the most powerful, non-invasive diagnostic tools available. Small, upward trends in this number often appear weeks before obvious physical distress or heavy coughing develops, giving the veterinarian a crucial head start if they need to change the current medication regimen or adjust medication dosages.
Chronic illness is not easy to deal with and rarely follows a straight, clear line. Managing a pet with heart disease takes patience and is more like a cross-country marathon instead of a sprint. Some months are beautifully stable, while others bring sudden setbacks, weather-related flares, or routine medication adjustments.
It is completely natural to feel overwhelmed or even a wave of guilt when a diagnosis is finally made. However, missing those initial, subtle, whisper-quiet signs doesn’t mean you failed your companion.
Long-term stability is entirely achievable, and with a dedicated veterinary team, consistent medication schedules, and proactive home monitoring, many dogs continue to enjoy good quality of life with their families for meaningful periods of time; often years after the initial diagnosis.

Diabetes in Dogs: More Manageable Than Most Owners Fear
Few diagnoses send dog owners into a tailspin faster than diabetes. The moment insulin injections enter the conversation, panic sets in. I understand why. Needles intimidate many people, especially when they imagine giving injections every day for the rest of their dog’s life. But after nearly two decades in veterinary pharmacy, I can honestly say that most owners adapt much faster than they expect.
Canine diabetes occurs when the body cannot properly regulate blood sugar levels due to inadequate insulin production or insulin resistance. The classic symptoms are increased appetite & thirst, more frequent and higher volumes of urination, and weight loss. Many owners first notice accidents in the house or a water bowl that suddenly needs refilling multiple times per day.
Left untreated, diabetes can lead to cataracts, nerve damage, urinary tract infections, and life-threatening diabetic ketoacidosis.4,5 I’ve worked with countless diabetic patients (both human and veterinary) over the years, and the people who succeed best usually develop consistent routines. For our pets, feeding schedules, insulin timing, monitoring appetite, and recognizing behavioral changes become second nature over time. Consistency in feeding and insulin administration is a key factor in successful diabetes management
One thing I always stress is that successful diabetes management happens over months and years, not days. Blood glucose levels fluctuate, and insulin requirements may change over time. Monitoring tools such as blood glucose curves can help veterinarians assess glycemic control and determine whether changes are needed. Some weeks go smoothly while others require adjustments, and that’s completely normal.
Managing this condition effectively relies heavily on the right prescription medications and routine monitoring, and your veterinarian may prescribe specific treatments to keep your dog stable. For example, Vetsulin is an intermediate-acting porcine insulin suspension commonly used for long-term management in dogs, while certain human insulin products, like Novolin N, may be used in canine patients under veterinary direction.5,6
In addition to insulin therapy, veterinarians frequently treat underlying or co-existing conditions that complicate diabetes. Since diabetic dogs are highly prone to secondary issues, you might find yourself managing conditions like pancreatitis or hyperadrenocorticism (Cushing’s Disease), and it’s easy to feel overwhelmed.
Unfortunately, some owners often blame themselves. They wonder if diet caused the disease or if they somehow missed warning signs. Canine diabetes is multifactorial, often associated with risk factors and health conditions that include genetics, age, obesity, hormonal disorders, and pancreatic disease. It’s rarely as simple as people think.

A Family Affair
Over the years, both our children have asked me to explain how certain medications help animals recover, and I’d like to believe that those conversations helped them understand responsibility, compassion, and patience in ways textbooks never could. A few families I’ve seen even involve older children in managing chronic diseases like diabetes, sharing the responsibility.
One of the most rewarding parts of veterinary pharmacy is watching frightened and concerned owners gain confidence. The family who cried during their first insulin lesson often becomes the same family comfortably managing injections six months later while their dog regains energy and stability. Something that has stayed true through my previous retail role in human products and now, as I help people care for their non-human family members, is the joy of seeing that shift from fear and apprehension to excitement and positivity when they realize they are helping their pets live happier, healthier lives.
