Foot pad damage

Foot pad looks like it is 90% torn off. What should we do for him?

You should take him to the vet, because I would go to the doctor if I had torn off most of the bottom of my foot. Foot pad injuries are no fun. He'll definitely need the wound to be cleaned out, and probably some stitches and antibiotics.
