You are making your dog scared when he has to go to the bathroom. Spanking is not going to make him learn where to go to the bathroom. Punishment in his crate will only make him scared of his crate?!!!?!!!? A crate is suppose to be a safe place for them to go and sleep, a happy place, not a cell for you to punish him in?!! Abuse will not help anybody or any pup learn where the right place to go to the bathroom is. When you catch your dog in the middle of peeing you need to say "outside" and take them out RIGHT away, no punishment......and if you dont catch him, simply clean it up with something that will remove his scent. If his scent is in the house that is where he will go. Take him out often, with treats in hand. The second he goes to the bathroom treat and praise, and if you keep doing this he WILL learn. Positive training, positive reinforcement HANDS FREE TRAINING works wonders. If he is scared to go pee because you punish him you will continue to have issues!

Excellent advice from Mika. May I add..dogs do not feel guilt, or right vs wrong. All he knows is you are yelling & smacking him for doing what every creature does. He may connect that you SEEING him do it makes you unpredictable, so he will hide the act somewhere else.
Keep him tethered to you at all times. Crate him if you need to take your eyes off him for even a minute. Take him outside at least once an hour, on leash. Get to know his signals. Get rid of the pee pads. They confuse the dog. Do not give him the whole house to pee & poop in.
Use an enzyme based cleaner, like Nature's Miracle, or SoX. Do not use an ammonia cleaner.

Definitely need positive reinforcement and possible to take her out every hour on hour for a week or so just to establish that is where she goes even if she doesn't have to go yet and then reward her when she does. Also clean areas where the dog has gone with either vinegar or natures miracle cause even a drop of a scent will keep a dog going to do his or her business in the same spot.
