My mom and I always check all those scary stories on Snopes.com (who are like the sentry dogs of the internet).

There are people out there who like to make up all sorts of scary stories to scare us and spread them in emails, which of course then spread like wildfire. Sometimes, these stories end up in reputable sources like your newsletter by well meaning people too (I heard this Swiffer story from my petsitter whom I trust with my life).

We don't believe anything until Snopes says it's true. I don't know why people would want to scare us puppies. Our lives are difficult enough (trying to get all the snacks and toys and naps we can!)

I'm 17 weeks old. Mom says I look like a crocodile. I have big dog teeth coming in and my puppy teeth haven't all fallen out. My vet (who's a dentist. Which means that he keeps telling Mom to brush my teeth every day. How awful is that?) says that I'm perfectly normal -- at least my teeth are!

I've been teething for about 3 weeks now. I can't wait for all of this to be over! Whew!

You said, " She knows it's wrong, yet she does it anyway. I know she can hold it, but she is choosing not to. "

It seems to me that you're reading a lot of human intent into your puppy's behavior. We puppies don't know right from wrong. Let's look at it from her perspective. She knows that if you catch her messing the floor or the sofa, she'll be punished. That's a big difference.

So she runs and hides from you if she can't wait long enough. And I don't think she's "choosing" not to hold it. Five minutes seems like a long time to me if you don't have a big bladder. The problem is that she's probably not alerting you early enough or strong enough at first for you to notice when you're busy cooking dinner or doing other household chores.
Initially, I howled and barked loudly at at my owners when I had to go outside. But I barked a lot at other times too. So as they worked on quieting my barking, my alerting to go outside got quieter too. So they had to become more attuned to what I was doing. I wanted to do what I was trained to do/what I get attention and treats for (what you would call the "right" thing) . But I need their help to do it. (They got me potty bells so they could hear me better).

Can you designate someone to put her out if she alerts when you're busy? Or maybe take her for a walk right before you start cooking dinner?

If not, then maybe crating while you're cooking dinner is the best option for you, and your pup who sounds like she's just trying to please you.
