UTI, again? STILL!!!

For what it is worth, Petey has suffered from this from a puppy. He has been on raw for about two years now. I have started to give him cranberry tablets and distilled water. He has been okay now for about six months,. I am hoping it is not just a coincidence.

Okay, last week we ended the 6 week course of antibiotcs...Toto spent the day at the vets again today with ANOTHER UTI. Well, actually he doesn't have any bacteria showing this time, BUT there are casts and he is peeing all thru the house again.
Good news, except for being slightly dehydrated, today's blood work shows NO kidney issues, yah hoo!!
But, he continues to become quite dehydrated at times almost daily, but is drinking well and plenty. He is on Cosequin and urinary acidifiers and his PH was fine, BUT, today's ultrasound showed possible issues with the bladder lining and shape. Kidney ultrasound also showed tiny kidneys, but since the bloodwork looked decent the vet felt those may be normal for him.
We are sending off his urine sample to IDEXX for a report on what type of cells the casts are and will go from there after that report, perhaps an exploratory surgery on his bladder.
This is getting VERY OLD! Not only is it a pain for me cleaning up after the peeing, but it also upsets him and he ends up spending a lot of time sulking under my desk.

I am so sorry...it's one thing to know there may not be something they can do, but it's a nother to try to figure out the whole thing so that he can get better. Sending PoP. I know how hard it is for both of you frownhug

Hmm, his ultrasound shows the same problems my male cat had. We ended up doing weekly fluids on him at home. His UTIs were never quite as constant as Toto's though. Now he's on completely wet and he gets maybe one UTI a year.

Toto's urine testing results came back, with not much help!!! The cells are non-typical cells, having various size and makeup and are suggestive of cancer or some other inflammatory process within the bladder or kidneys.
Next step is send ANOTHER sample in for a more specific type of test (read VERY expensive) or do another ultrasound in a couple of weeks to see any changes.
Since Toto's problem is chronic I really can't believe he has cancer, even though his age and symptoms point that way, so my vet and I have decided to do another ultrasound and urinalysis (in house) next Thursday. IF there is any bacteria present in the sample we will try long term antibiotics. If the casts are still present with no bacteria and/or there are more visible changes within the bladder or kidneys, we will reconsider sending more samples out for further testing.
There aren't lots of treatment options if he does have cancer so...
