Has she been to a vet who's checked her out?
If you don't have an answer from your vet, have you checked her diet?
If you're feeding her food with grains in it, that could be causing some of the problems. Wheat and corn and other grains can really cause skin problems for dogs.
Mikey is allergic to chicken. I introduced a chicken based (grain free) food into his lamb based food and within a few weeks he started scratching like crazy. After I eliminated the chicken based food, a few weeks later, no more scratching.
If you don't get answers from your vet, start looking at what you're feeding her.
Also, if you're bathing her a lot to alleviate the scratching, be careful. That could be exasperating the situation and you don't want to strip her coat of it's natural oils.

Mikey really nailed this question. Start with the food, check for fleas and bathe in a good shampoo and if all else fails go to the vet. Nice Post Mikey
