Yes, she needs to see a vet. That's much too old for baby teeth, and small, flat-faced breeds (like the Lhasa) are very prone to gingivitis and tooth decay because their little mouth is so crowded. The majority of dogs over 3 (of every breed) have gum disease, which leads to bone loss in the jaw, which causes teeth to become loose.

Hopefully it's not too late to save the rest of her teeth, she should have a full dental cleaning (under anesthesia) and have any more loose ones taken out. Mouth problems can be very painful and lead to infection in other parts of the body when germs enter the bloodstream there.

Teeth problems are very common in small breed dogs. Since all dogs have the same number of teeth, small dogs have less room to hold all the teeth. In turn these teeth are more likely to have be loosened or have gum disease. Some cases there is not much you can do. It just happens. Brushing and giving something like greenies to keep the teeth strong may help to delay the problems. I have a small dog that is only 8 and had to have a lot of his teeth taken out when he was younger just because they never grew in right. He is fine. He even still eats hard dog biscuits.
