Have Spencer's blood tested for levels of pheno in his blood and liver. Pheno is a very strong med and though it is usually given to start at a low dosage, it might be something that needs to be adjusted for him.

The joint pain is something relatively unusual for a side effect of pheno, but it's entirely possible. Ask the vet to check him for any kind of neck injury that might have happened concurrently as he started the pheno. If you don't get a satisfying answer, find a second vet for a second opinion.

I had a dog who was on pheno and it can cause a host of side effects (as can it's more modern counterpart, potassium bromide) so it's best to monitor closely any changes that occur with these meds.
