You need to set up a schedule and take her out after she wakes up, after each meal, before she goes to bed for the night... Basically ever hour. She either needs to be leashed to you so you can watch her every second to see if she is showing signs of needing to go out or in her crate when you cannot watch her. Take her straight out on a leash (RUN if you have to!) to her potty spot and really praise her and give her treats EVERYTIME she goes to the bathroom outside. Clean up accidents with enzymatic cleaner.

As far as grabbing her collar.. NEVER GRAB HER BY HER COLLAR FOR PUNISHMENT! She is head shy and needs desensitizing. Touch her tail gently and give her a really tasty treat. Work your way forward slowly. Go back a step if she is uneasy. You should be able to touch her collar eventually with lots of practice. Taking her collar should mean fun things like walks
