You'll need a kennel that the dog can sit, lie down, and stand up and turn around without showing any signs of discomfort.

Thats a long ride so ask your vet if there is anything you can do to help with car sickness if it ends up happening. If the ride is longer then 3 hours put some food in a bowl (there are dished they sell that hook to the door) make sure he has access to water, put some puppy pads and towels/blankets in there for comfort.

If the puppy does not have his shots DO NOT take him anywhere! He could catch a diesaese.
